Your First Visit Checklist

We want you to feel comfortable at Affiliated Retina Consultants. Here are some important things to know in preparation for your first visit.

 

  • Print and fill out all necessary forms before your visit. 

  • Plan for a minimum of 1.5 hours for your first visit with dilation.

  • Bring a list of all medications you are currently taking.

  • Have your photo identification card and current medical insurance card(s) available.

  • If your insurance requires co-payments, we accept cash, check, or credit cards.

  • If your insurance requires authorization for you to see a specialist, please obtain an authorization
    from the appropriate source prior to your visit.

  • Bring sunglasses (rain or shine) because your eyes will be dilated.

  • If possible, arrange for someone to drive you home.

  • Again, please bring all requested items with you to your appointment.

Insurance and Billing

Affiliated Retina Consultants participates with most insurance companies.  

 

If you have any questions regarding our participation with your medical insurance carrier, please contact our office or speak with your insurance company directly.

If your insurance company requires that services be authorized prior to your first visit, you are responsible for obtaining that referral from the appropriate source.

We will bill your primary and secondary insurance carriers directly.  You will only be billed the portion of your charges that your insurance company does not pay.  
If your insurance company pays you directly, please contact our patients accounts department and/or send the endorsed check to us so we can apply it to your balance.

We are pleased to offer you the convenience of paying your account balance by cash, personal check, VISA, MasterCard, or money order. 

 

Credit Card payments can be made over the phone.

 

We also have payment plan options.
